Lead APP · Austin Urology Institute

Cysto. TRUS. Flow.
Run it the same way every time.

I'm Alex Hughes, MSN, APRN, FNP-C. I build standardized BPH diagnostic pathways: cystoscopy, TRUS, uroflow/UroCuff, and PVR, done the same way for every patient. A repeatable diagnostic system is what gets the right patients to the right procedure, whether that's Aquablation, UroLift, or something else. Surgeons do the procedures. I build the system that feeds them, and I teach other practices to do the same.

About

A prostate size tells you something. It doesn't tell you everything.

I'm a procedural urology APP and Lead APP at Austin Urology Institute in Austin, Texas. Day to day that means BPH/LUTS, elevated PSA, hematuria, stones, OAB, and men's health, plus the office procedures that go with them: cystoscopy, TRUS prostate sizing and biopsy, UroCuff testing, bladder Botox, Axonics PNE evaluations, and catheter/stent procedures.

My second focus is OAB and incontinence: Axonics sacral neuromodulation (including PNE evaluations), eCoin tibial neuromodulation, bladder Botox, and Bulkamid.

A good BPH workup puts symptoms, PVR, flow, anatomy, medication history, and cancer risk together before anyone talks about a procedure. That workup is where APPs make the biggest difference, and where a lot of practices leave capacity on the table.

In 2026 I'm the APP Program Lead and faculty for AUI's BPH & Aquablation Program site-visit education. I co-facilitate a paired surgeon–APP model with Koushik K. Shaw, MD: he covers operative decision-making and technique, and I cover what happens before and after the OR: patient identification, objective evaluation, office diagnostics, patient education, and long-term continuity.

Before nursing, I worked as a firefighter/paramedic. That's still where a lot of my bias toward clear systems and calm handoffs comes from.

Expertise

A repeatable diagnostic system decides who gets which procedure.

Cystoscopy, TRUS, and flow testing, standardized so every BPH patient gets the same complete workup. Plus OAB and incontinence care.

01

BPH & Aquablation diagnostics

Cystoscopy, TRUS, uroflow/UroCuff, and PVR, built into a standardized pathway that runs the same way for every patient. That's what gets the right patients to the right procedure, Aquablation and UroLift included: candidacy, procedure selection, counseling, the handoff to the surgeon, and post-op care. I teach practices how to build it as a PROCEPT clinical educator and as faculty for AUI's BPH & Aquablation Program site visits. Surgeons perform the procedures; I build the diagnostic pathway around them.

02

OAB & incontinence

Axonics sacral neuromodulation, including PNE evaluations; eCoin tibial neuromodulation; bladder Botox; and Bulkamid. Getting patients past failed medications and onto the next appropriate therapy.

03

BPH evaluation & pathways

Standardized workups built on IPSS, PVR, PSA density, medication reassessment, cystoscopy/TRUS, and flow testing, followed by shared decision-making, procedural triage, and post-op surveillance.

04

Procedural urology

Office cystoscopy, TRUS sizing and biopsy, UroCuff, bladder Botox, Axonics PNE, and catheter/stent procedures, plus procedural selection and postoperative management.

05

APP training & development

Training and onboarding APPs and clinical teams in procedural readiness, workflow efficiency, patient counseling, and knowing when to escalate to the surgeon.

06

Service line efficiency

APP diagnostic ownership that widens appropriate procedural access and protects surgeon time for the OR, with clear referral workflows and a clean clinic-to-OR handoff.

The BPH pathway, start to finish

  2. DiagnosticsUroflow / UroCuff, PVR, cystoscopy, TRUS
  3. SelectionProcedure matched to anatomy and goals
  4. HandoffCoordinated referral to the treating surgeon

Topics available to book

Standardizing BPH diagnostics

Cysto, TRUS, and flow as one repeatable system, and how it feeds the right patients to Aquablation and other procedures.

Beyond meds in OAB

Where Axonics, eCoin, Botox, and Bulkamid fit, and how to build the referral path.

Building an APP-led BPH pathway

How to structure the workup so APPs own evaluation and diagnostics, and physicians spend their time where it counts.

The BPH workup, done right

IPSS, PSA density, flow, PVR, cystoscopy, and TRUS: what each tells you, and what it doesn't.

Procedural selection for BPH

Matching patients to the right procedure based on anatomy, symptoms, and goals, from the APP's seat.

Training urology APPs

Getting new APPs procedurally competent and productive, and keeping them.

Work with me

Put APPs in charge of the workup. I'll show you how.

I help practices put APPs in charge of diagnostic BPH care (the workup, the office diagnostics, the handoff to the surgeon), and I work with companies, educators, and podcast hosts who want that perspective.
